A lifestyle change and support program for individuals living with chronic diseases.
This program is intended for people who have been diagnosed with/or at risk for:
- Heart disease
- High cholesterol
- Diabetes (gestational, type 1, type 2, pre-diabetes)
- High blood pressure
The program is intended to help individuals gain knowledge about their disease, discuss health concerns, and learn how to manage/control their health. The program offers one-on-one appointments and group chats on various topics (such as nutrition, blood pressure, stress management, etc.).
This clinic does not see patients for anything other than cardiac and diabetic care.
