Provides client-directed delivery of care with a comprehensive, assertive outreach model for people with multiple complex needs. These services are available in Kelowna and Kamloops
Assertive Community Treatment Teams help you with a team-based care approach. If you are experiencing multiple complex needs and mental illness and/or substance use concerns, our teams provide you with client-directed care, assertive outreach and comprehensive, continuous service delivery.
Services may include: Assistance to find & keep housing, Primary Care, Outreach, Money management & employment, Psychosocial rehabilitation, Court liaison, and the following Treatment, Support & Recovery Services.
Admission Criteria:
- Adults (18+)
- Severe Mental Health Disorder (Schizophrenia, Schizoaffective Disorder), and possible Substance Use issue
- Inability to participate in traditional office-based or group services
- Significant functional impairment e.g. Inability to maintain safe and consistent housing
- Able to function safely in the community with enhanced supports
- High use of hospital services (e.g. high number of ER visits)
- Criminal justice involvement (e.g. frequent incarcerations)
- Complex comorbidities
Referral accepted through: Kamloops Community Mental Health & Substance Use Services Central Intake and Kelowna Mental Health & Substance Use and King Street Centre.
