Northern Health long-term care homes strives to adhere to a person and family centred care model. It is a partnership between the person we serve in health care system, their family and their care providers. DementiAbility is a program that embodies these qualities. NH has developed a learning module to educate community members on how these person and family centred care principles are applied to a person with dementia.
Accessing This Service
Individuals who may require health care or personal support services can be referred to Home Health for an assessment of their needs. A referral can be made by the individual, a family member, the hospital, a friend or community group, or the family doctor. If you or a family member needs a Home and Community Care Service, please contact the local office near you. A professional staff member will assess the patient's situation, need and eligibility. A Home Health Case Manager will work closely with you, family and friends to:
- Determine eligibility for services
- Assess your needs
- Determine what kind of services are needed
- Determine whether there will be any costs involved for certain services, plan, coordinate, and monitor several care services.
