Provides a variety of services to support seniors in being independent, informed and socially connected in our community.
The OneSky Community Connector for Seniors Services provides one-on-one support for high-risk seniors (65+) in accessing timely community services to address isolation, frailty, and vulnerability.
Once a physician refers their qualifying patients to the Community Connector for Seniors Services, they will receive regular updates on what services they are accessing and any other information that the Connector finds relevant to their healthcare.

Eligibility: Older Adults 65+ living in the Penticton, Summerland, Naramata, Kaleden areas who may benefit from support with::
- Social isolation / loneliness
- Emotional wellbeing
- Loss of confidence / purpose
- Finances, housing, and accessing benefits
- Life changing events such as retirement, bereavement, changes in health status and independence
- Healthy lifestyle choices
- Getting out and about
- Caregiver stress
