Provides a variety of services to support seniors in being independent, informed and socially connected in our community.
The OneSky Community Connector for Seniors Services provides one-on-one support for high-risk seniors (75+) in accessing timely community services to address isolation, frailty, and vulnerability.
Once a physician refers their qualifying patients to the Community Connector for Seniors Services, they will receive regular updates on what services they are accessing and any other information that the Connector finds relevant to their healthcare.
The difference between the Community Connector and OneSky’s other navigation services is the ability to liaise with the primary care provider. The Community Connector will request client consent in order to provide updates to the health care team (primary care providers, case managers, etc.) and vice versa, creating a team-based approach to ensure the individual’s overall well-being.
