Offers a wide range of home and community care services.
Home and Community Care provides a range of health care and support services for eligible residents. Home and community care services are designed to provide the right care in the right setting by offering:

  • At-home services (for example, home care nursing) to prevent admission to hospital or to allow for earlier discharge from hospital
  • Respite (temporary relief) for a client’s unpaid caregivers
  • Assisted Living services to clients who are still independent, but require a higher level of care
  • Residential care services to clients who can no longer be supported in their homes
  • Hospice palliative and end-of-life care that supports people at the end of life, and their loved ones

    Home Health Service may include:
  • Acquired brain injury program
  • Adult day programs
  • Choice in supports for independent living (CSIL)
  • End of life care / Palliative care
  • Home care nursing
  • Home Oxygen Program (HOP)
  • Medical Assistance in Dying(MAiD)


    Other services may include:
  • Case Management: Case managers act as coordinators to help eligible clients obtain home and community care services.
  • Community rehabilitation: a professional service, delivered to eligible clients in the community by rehabilitation therapists who provide assessment and treatment to ensure a client’s home is suitably arranged for their needs and safety.
  • Health services for community living (HSCL): provides non-emergency nursing, rehabilitation, dietary and dental hygiene services to adults who live in the community and have a developmental disability and are eligible for services under Community Living British Columbia (CLBC).
  • Home support: Home support services are designed to help eligible clients remain independent and in their own home as long as possible. Home support provides personal assistance with daily activities, such as bathing, dressing, grooming, and medication assistance
  • Lifeline: Lifeline is an easy-to-use medical alarm service designed to reduce the risk of living alone.
