Home Care Program
End of Life Care / Palliative Care, Home Health Care, and Indigenous Services
Provided by Carrier Sekani Family Services
Provides in home care and support to member First Nations based on individual medical needs. For on-reserve individuals dealing with chronic disease, end of life care, or returning home from a hospital stay.
The Home Care Program provides a range of home and community care services to Carrier and Sekani community members living on reserve. This includes members of Wet’suwet’en, Burns Lake Band, Nadleh Whut’en, Cheslatta Carrier Nation, Nee Tahi Buhn, Yekooche First Nation, Skin Tyee, Stellat’en, Takla Lake First Nation and Saik’uz First Nation.
This program mostly provides services to adults in the 30-59 and 60+ stages of the life cycle. However, they will provide their services to anyone in need at any age. Services are provided free of charge.
The home health care team includes Residential Care Aids, doctors, physiotherapists and Community Health Nurses and Mental Health Clinicians who work as an integrated team with CSFS Mobile Diabetes staff. They also work collaboratively with each band-funded homemaker program.
Service offered include:
- Providing support to individuals requiring heath aids and medical safety equipment, and assessing individuals for long-term care requirements if needed.
- Regular personal care assistance
- Homemaker services are not provided directly. However, collaboration takes place with each band-funded homemaker program, which offers services such as cleaning, cooking, firewood collection, and grocery shopping, etc. Requests for these services should be directed to the respective band.
Personal care services consists of:
- Personal hygiene
- Hospice/palliative care (end of life)
- Diabetes management & diet advice
- Blood glucose checks
- Blood pressure checks
- Massage
- Mobility/exercise
- Socialization
- Terminal illness care
- Respite
Referrals are accepted from community members and professionals, and followed by an individualized assessment to determine specific needs. For more information, take a look at the program brochure here.
