Youth Intensive Case Management - Youth - Interior Health Area

Provides intensive case management services to high needs clients with substance use and mental health issues.
Our Youth Intensive Case Management teams offer low barrier outreach services to meet you where you are at, without judgement. We will work with you to meet your immediate needs and connect you with supports and services to improve your health and meet your wellness goals.

Admission Criteria:
  • Ages 12-24
  • Severe Substance Abuse with a Mental Health Disorder
  • Inability to participate in traditional office-based or group services
  • Significant functional impairment e.g. Inability to maintain safe and consistent housing
  • Able to function safely in the community with enhanced supports


    Additional Criteria:
  • High use of hospital services (e.g. high number of ER visits)
  • Criminal justice involvement (e.g. frequent incarcerations)
  • Complex co-morbidities
