Home & Community Care Program - Merritt

Provides a range of health care and support services for eligible residents.
Services include:
Foot Care Program: The Program has a clinic every Wednesday in the Home and Community Care Department. An assessment must be done by a Registered Nurse on the client prior to admission to the program. Clients must book appointments for foot care. Foot care can be done either at home or in the clinic. This program benefits any client with diabetes, toenail problems or other disease processes that affect the feet.

Frozen Dinners Program: Provides meals for Clients who are nutritionally at risk, post-operative clients that need short term assistance with meals and elderly clients who have difficulty preparing meals.

Dinners at Home Program: Assist those in the communities who are nutritionally at risk, shut-ins and or have health issues preventing them from cooking their meals from scratch. The IHA Frozen Meal Program supplies these meals. The meals are prepared in Penticton, BC, under the strict guidelines of the Nutritionist /Dietician and then purchased by Scw’exmx and delivered once a week to Community members. The home and community care nurse conducts a needs assessment in order to access this program.

Health Teachings: Home or office visits with clients to help them understand their health care conditions and resources that are available.

Medic Aid Response Program: Provided benefits for clients who have medical issues that put them at risk for a fall, clients who live on their own with chronic health conditions that put them at risk for an event. Medic Alert monitors are installed with no monthly cost to the client.

Medication Administration and Management: Medications are prepared, and administered through various routes, by the Home Care Nurse with orders from the physician. The HC nurse observes the medication side effects, educates the client, reviews the medications and reports any untoward signs and symptoms to the physician.

The Nicola Valley Aquatic Centre: Provides access to health & wellness Swim & Gym for eligible clients. Available to all Coldwater, Nooaitch, and Shackan members living ON reserve (within these three communities). MUST Phone in to SCHSS to register (only registered individuals will have free entry). Pool Hours: Monday, Wednesday, and Friday, Early Bird 6:00 AM - 8:00 AM, Public Swim/Programs 9:00 AM - 1:00 PM, Public Swim/Programs 3:30 PM - 8:30 PM

Quarterly Nurse Visits: Visits done by the RN or the PCA on clients with chronic conditions that require monitoring for therapeutic intervention but are medically stable.

Supportive Care: Supportive Care is offered to Palliative clients, Elders with chronic health conditions and disabilities that require personal care assistance with bathing and foot care, meal assistance and assistance with accompanying clients to visits with their family doctors as a nurse advocate.

The Self-Management Program: for Chronic Conditions helps individuals living with chronic diseases build confidence in managing their health. It offers workshops led by trained facilitators, covering topics like pain management, medication use, nutrition, exercise, and communication with healthcare providers. The program aims to enhance quality of life through education, skill-building, and peer support.

Wound Care: Wound care is available to clients at home or in the office with a referral from a Medical Practitioner.

Assisted Living: Personal Care done by PCA, such a bathing, foot care, hair care, dressing, frozen dinners at home for clients who are nutritionally at risk
Also serves Community members from Coldwater, Nooaitch, and Shackan bands.
