Home Care Nursing: Registered Nurses deliver in-home care for Elders, individuals with chronic illnesses, and those recovering from illness or surgery. Services are culturally sensitive and designed to support independence while meeting health needs.
Foot Care Program: An assessment must be done by a Registered Nurse on the client prior to admission to the program. Clients must book appointments for foot care. Specialized foot care is available for community members, including nail care, callus removal, and education on foot health—especially important for those living with diabetes or circulation issues.
Frozen Meal Program – Dinners at Home: Nutritious frozen meals are prepared and delivered to clients who may have difficulty cooking, helping maintain a balanced diet.
Dinners at Home Program: Assist those in the communities who are nutritionally at risk, shut-ins and or have health issues preventing them from cooking their meals from scratch. The IHA Frozen Meal Program supplies these meals. The meals are prepared in Penticton, BC, under the strict guidelines of the Nutritionist /Dietician and then purchased by Scw’exmx and delivered once a week to Community members. The home and community care nurse conducts a needs assessment in order to access this program.
Health Teachings: Home or office visits with clients to help them understand their health care conditions and resources that are available.
Medic Aid Response Program: Support the use of personal medical alert devices to ensure clients can quickly access emergency help when needed.
Aboriginal Diabetes Initiative: This program reduces the risk and impact of Type 2 diabetes through culturally relevant education, prevention, and health promotion. Services include blood testing, eye exams, diabetic nurse educator visits, orthopedic and mastectomy clinics, food skills courses, and monthly walking/vitals group sessions.
Nutrition Counselling: Clients can meet with health professionals for personalized nutrition guidance to support chronic disease management, weight control, and overall wellness.
Case Management Team coordinates health services, referrals, and resources for clients with complex needs, ensuring they receive comprehensive, ongoing support.
Medication Administration and Management: Assists with safe and accurate medication administration, including reminders, monitoring, and coordination with prescribing physicians.
Chronic Disease Self-Management: empowers clients to better manage chronic conditions through education, goal-setting, and regular follow-up care, helping reduce complications and improve quality of life.
Wound Care: Skilled nurses provide assessment, treatment, and monitoring for acute and chronic wounds, promoting healing, preventing infection, and educating clients on self-care techniques.
Programs are all free for all eligible residents. Only serves Community members from Coldwater, Nooaitch, and Shackan bands.
