Provides comprehensive and collaborative care coordination services for people with multiple complex needs.
Available in Cranbrook, Kamloops, Kelowna, Kaslo, Nelson, Penticton, Vernon and Williams Lake with a physician referral.

Admission Criteria:
  • Adults (18+)
  • Severe Substance Abuse with a Mental Health Disorder
  • Inability to participate in traditional office-based or group services
  • Significant functional impairment e.g. Inability to maintain safe and consistent housing
  • Able to function safely in the community with enhanced supports

    • Additional Criteria:
  • High use of hospital services (e.g. high number of ER visits)
  • Criminal justice involvement (e.g. frequent incarcerations)
  • Complex comorbidities
